Next Steps:

  1. Submit my yellow pre-enrollment form to let Midvale know we have a kindergartner coming.
  2. Enroll my student at the school. Submit immunization records and any needed documentation to the office.
  3. Complete online Registration on Skyward for my student in August during the K-12 registration window.
